(FULL TIME, REMOTE) CARE COORDINATOR / CARE ADVOCATE

If you have a professional background working with seniors and working in this field is your passion, contact us! We are looking for Nurses, Gerontologists, Social Workers, Recreation Therapists, Occupational Therapists, and others with a minimum of 2-3 years of expertise in Senior Care.

 

AGING LIFE CARE MANAGEMENT RESPONSIBILITIES:

 

  • Performs a holistic, person-centered comprehensive assessment to evaluate psychosocial, cognitive skills set, home safety, medical history/current status, and level of care. 
  • Develops, coordinates, and monitors clients’ care plan and evaluates the plan’s effectiveness ongoing. 
  • Serves as a liaison between ancillary services to advocate for the client. 
  • Coordinate client’s physician, dental, and other medical appointments.
  • Collaborates with healthcare professionals in different settings to advocate for quality of care.
  • Documents daily care management activity in the electronic chart.
  • Communicate with family members by phone or email after each visit.
  • Demonstrates a high level of competency specific to discharge planning. 
  • Ability to link clients to local resources/programs/providers/facilities to support client's needs. 
  • Provides consultation with clients in the selection of appropriate living communities, attends tours, and facilitates the transition and move into the facility. 
  • Collaborates with key team members regarding the client’s healthcare in overcoming barriers in meeting goals and objectives. 
  • Consults with Director and other team members regarding crisis intervention.
  • Advocate for client needs at home or in a facility.
  • See clients in their homes, assisted living, skilled nursing, and hospital.

 

MARKETING / INTAKE RESPONSIBILITIES:

 

  • Take potential client intake calls to determine needs, 
  • align them with appropriate services, and complete paperwork to start services.
  • Attract referrals and inquiries to build care management services.
  • Contact potential and existing healthcare referral sources on behalf of Aging Care Management.
  • Promote Aging Care Management services to medical, financial, legal, professional, civic, community, and Alzheimer’s related groups 

 

SKILLS AND EXPERIENCE REQUIREMENTS:

  • Personal and compassionate service – focused on the individual client’s needs
  • Working knowledge of the aging process including diagnosis, psychosocial issues, medical procedures, and medications. 
  • Knowledge of physical, emotional, social, leisure, and spiritual needs of older adults. 
  • Well-developed communication skills (both verbal and written) 
  • Interpersonal communication skills; 
  • Thorough understanding and use of medical terminology. 

 

REQUIRED EDUCATION & EXPERIENCE:

  • A Master’s degree is desirable, especially in the fields of Gerontology, Social Work, Nursing, OT, and PT. 
  • At least 2-3 years of experience in direct care or care management with geriatric patients. 
  • Proven clinical expertise in the areas of aging and dementia. 
  • Familiarity with working with intergenerational family systems.
